Health Dept. - Public Health Nursing1971 State Street Columbus, IN 47201 ServicesBlood pressure/Blood sugar/Total cholesterol Screening Clinics are held the first, second and third Monday morning of the month at the State Street site from 8:00 – 10:00AM. The cost is $5.00 for Blood sugar testing (under 55) and $10.00 for Total cholesterol/Blood sugar testing. Older Adult outreach clinics are held monthly at various locations throughout the community. Services offered at the outreach clinics include blood pressure, hemoglobin, cholesterol, blood glucose screenings, weight and diet counseling . Cholesterol and Lipid profile Screening is offered the fourth Monday morning of the month at the State Street site by appointment. The cost if $20.00. Communicable Disease Investigation is to prevent epidemics and the spread of disease. Information and communicable disease reporting can initiate from laboratories, hospitals, doctor's offices, and other health departments. Surveillance and investigation activities involve communicating with the infected individual; his/her contacts and medical care provider(s). Mandated Reports are filed with Indiana State Department of Health (ISDH). Foreign Travel information is provided to persons traveling outside the United States for disease prevention, recommended immunizations, and where to get the recommended immunizations. Immunization for Tetanus-Diphtheria, Hepatitis A/B, and Hepatitis A are offered by appointment along with Healthy Travel Tip information packets and country specific handout advisor sheets. Health Education programs offered include HIV/AIDS, Hepatitis, Sexually Transmitted Diseases, and the role public health plays in regards to health maintenance and disease prevention. Information, presentations, and printed materials are available upon request. HIV Counseling and Testing clients are seen anonymously or confidentially by confirmed appointment. HIV/AIDS education is tailored to address each individual's risk factors for HIV infection. An appointment may last 30-60 minutes. Immunization Program/Clinic's focus is to provide clients with immunity against specific diseases. Immunizations against vaccine preventable diseases for infants, children, and adults are available. Immunization clinic for infants is held every Thursday by appointment. Immunization clinic for older children and adults is held every Monday and Friday by appointment. A record of previous immunizations is required for children 18 and under. Lead Testing is available for children 0-6 years of age. Childhood lead poisoning is a major, preventable environmental health problem in the U.S. High lead levels can cause major health and educational problems. Public Health Nursing (Home Visits) for high-risk elderly and infants are a priority for our department. By monitoring a patient's health and assessing home situations individuals can possibly remain in his/her home. Low birthweight infants are evaluated for health and development status, and parents are offered education and assistance as needed. Home visits for assessment, medication setup, and health monitoring are carried out by a Registered Nurse on an as needed basis. Referrals accepted from MDs, other agencies, and individuals. School Health Program provides health-related education and services to private and parochial schools. Mandated screenings for hearing, vision, scoliosis, height/weight and blood pressure are provided. Assistance with immunization surveys, health appraisals and health education to students and counseling services to teachers, students, and parents are provided. Students requiring follow-up are referred to their private health care providers. Sexually Transmitted Disease (STD) Program/Clinic services include Sexually Transmitted Disease Screening, diagnosis, treatment, and risk reduction counseling. STD Clinic is held every Tuesday and Wednesday by appointment. All counseling and provider referral services are confidential. Assistance can be provided to minors without parental consent. Tuberculosis (TB) Clinic maintains a surveillance system and TB case registry to ensure prompt reporting. Tuberculin skin testing is usually available Monday, Tuesday and Friday, the cost is $5.00. TB medicine (provided by the ISDH) is dispensed when ordered by the client's doctor and clients are seen on a monthly basis by a RN for an evaluation regarding medication compliance and effects. WIC (Women, Infants, and Children) Program is a short-term intervention program designed to influence lifetime nutrition and health behavior in women who are pregnant, postpartum, and/or breastfeeding, and to children who are under 5 years of age. The U.S. Department of Agriculture sponsors WIC. Participants must live in Bartholomew County and be at a nutritional or health risk and financially eligible to qualify.
