How do I request a birth certificate through the mail? |

Birth certificates may be requested through the mail using our printable birth certificate application. After completing the application, please include one form of signature identification and correct fee. The fees are listed at the bottom of the application. We try to send out the birth certificates within a day or two after we receive the request. If requesting a birth certificate to be sent back to you overnight mail, you must include envelope with postage already paid. You may also request a birth certificate using a credit card by going to www.vitalchek.com.

How do I request a death certificate through the mail? |

- Death certificates may be requested through the mail using our printable death certificate application. You must include the fee, which is $8.00 for each copy, and all of the requested information which is listed on the application. You may also request a death certificate using a credit card by going to www.vitalchek.com.

are located on the corner of 3rd and Franklin Streets across
from the Crump theatre on the 3rd floor.
If traveling
from I-65: Take SR 46 east exit to 2nd Street in downtown
Columbus. Turn left onto Franklin St. and we are on the left

Who can obtain a birth or death certificate? |

Death
Certificate: Anyone who has a direct interest can receive
a death certificate after filling out an application for a
death certificate
Birth
Certificate: With proper identification a birth certificate
may be requested by immediate family such as:
- Father
- Mother
- Brother
- Sister
- Child
- Aunt
- Uncle
- Grandmother
- Grandfather
- Stepmother
- Stepfather

Can a birth or death certificate be faxed? |

Birth
and death certificates would not be certified and therefore
would be unacceptable to state and local agencies if faxed.
