Vital Records Programs
- Birth Records -
Record and issue certified copies of birth certificates for persons
born in Columbus and Bartholomew County to self or immediate family
members. Fee: $8.00 for regular size and $10.00 for sleeved or
wallet size.
- Death Records - Record and issue certified
copies of death certificates for a person whose death occurred
in Columbus and Bartholomew County. Fee: $8.00 for each copy.
- Birth and Death Statistics - Statistics
are available concerning various aspects of births and deaths
in Bartholomew County.
- Genealogy - Birth and death records have
been recorded since 1882. Genealogy information is available to
the public.
- Statistics - Prepare and report records
of births, deaths, and total deaths (stillbirths) to the Indiana
State Department of Health. Prepare monthly a death report for
the county auditor, circuit court clerk, and other agencies.
- Burial-Transit Permits - Issue a permit
for disposition of human remains prior to any type of disposition
of a body and prior to removing a body from this county.
- Adoptions - Change records for adoptions
(No Fee).
- Legitimations - Legitimize records. Parents
decide to marry after a child is born and change the child's'
name to the father's name and add the father to the record. Fee:
$20.00.
- Paternity - Change records as required
by court order determining parentage. No fee if ordered by the
court.
- Record Changes - Amend records as required
by the Indiana State Department of Health and requests from individuals
as approved by this department. Fee: $20.00 for paternity affidavits
and $20.00 for Affidavit of Amendment
